SYMPTOMSWhen you create one of the following, Word does not add the additional tabs in the compiled table of contents or index:
CAUSEWord tries to eliminate the possibility of awkward or inappropriate spacing in your compiled table of contents or index, especially when you have set custom tab settings. Word removes those tabs from your table of contents and index entries.

Jane <tab> Austen's <tab> Worksafter you create your table of contents, the heading appears in the table of contents as follows:
