 SUMMARYThe following procedures allow information to be entered into a template form in Microsoft Word for the Macintosh without modifying the template form. When creating a template form document, the space for user-entered information should be identified by a series of symbols -- for example, ****. To have an entry underlined, select the series of symbols and format them as underlined. To enter information into the form, use the Find command to scroll through the document: 
 MORE INFORMATIONIf the document has been locked, Word will prompt you to save it with a new name when you try to close it. After the document is saved, do the following to prevent modification to the designated form document:
