To get a unique VxD identifier, you need to fill out the form below and return it to Microsoft. The form can be sent to the Internet address: vxdid@microsoft.com.

Another way to get an identifier is to fax this information to Windows DDK Product Support, at 1-206-93MS-FAX or mail the information to:
