The locking mechanisms used in the ancillary function driver (AFD) are
insufficient, resulting in one thread of execution canceling the I/O
request packet (IRP) that was used by another.
RESOLUTIONTo resolve this problem, obtain the hotfix mentioned below. With the hotfix, the AFD driver has been modified to ensure that access to I/O request packet under the above-described conditions is properly synchronized. STATUS
Microsoft has confirmed this to be a problem in Windows NT versions
3.5, 3.51, and 4.0.
