Select the item on which to base a form

Each Outlook item is a collection of fields. Some of these collections of fields may be more useful for particular applications than others. To see the complete list of fields for an item, click All in the Field Chooser. For example, to see all the fields in a mail message, click All Mail Fields. If the field you want is not listed, you can create a custom field by clicking New in the Field Chooser.

Mail Message Send a form to other people with an associated Reply form so they can send information back.
Post Send a form to a folder.
Appointment or Meeting Request Record scheduling information about people, dates, places, and times for an appointment or meeting.
Contact Record information about a person or organization.
Task or Task Request Record a personal or work-related duty or errand that you want to track through until completion.
Journal Entry Record interactions with important contacts, record items or documents that are significant to you, and record activities of all types.
