User <user> already exists

This error has the following cause and solution:

• You attempted to add a user with a username that already exists.  You may want to choose another name; for instance, if there is already a Joyce, add JoyceF.

Note   Names are not case-sensitive, so if you already have a Joyce, adding JOYCE generates this error.
